To Request an Appointment with FOMC

To Request an Annual Fly Periodic Health Assessment 

First, complete your PHA questionnaire in MyIMR then email our org box at usaf.barksdale.2-mdg.mbx.bomc@health.mil with your availability. Please provide three dates and times.  

Fly PHA appointments are Monday, Tuesday, Wednesday, and Friday, 8-10 a.m., in 30-minute increments. You will receive a call or an e-mail with your appointment time.  

Please email BOMC through its org box at usaf.barksdale.2-mdg.mbx.bomc@health.mil for the following:  

• HIV lab orders 
• SHPE appointment scheduling 
• Fly PHA scheduling 
• Questions or concerns involving medical clearances, separation physicals, deployment clearances and initial flying class exams 

To Request an Initial Flight Physical, Re-Training, Overseas, or other Medical Clearance

• Go to the AF Portal
• Type in My IMR
• Click Medical Clearances
• Click start new Medical Clearance Request
• Select the type of medical clearance you are applying for and follow the instructions
