Referral Management Center (RMC)
You can check the status of your referral at www.humanamilitary.com or by calling Humana at 1-800-444-5445 after 72 hours.
Your referral must be within 30 days of the expiration date before it can be renewed. Please contact your PCM at (318) 456-6555 or via Secure Messaging to initiate the process.
Automated Calls About Your Referral
Once your referral is approved, provider is assigned, and appointment is scheduled, please contact the Referral Management Center (RMC) at 318-456-6562 with the following information:
- Provider Name and the Date and Time of the Initial Appointment only
Note: please call RMC if you do not wish to use the referral.
Access to Care
If your specialty care provider is unable to book your first appointment within 28 days, please contact Humana at 1-800-444-5445 to request assistance in finding a different provider who can meet your needs.
